Portsmouth Southcoast Primary Care Network is a collaboration of the Trafalgar and Craneswater Group Practices, based in the South of Portsmouth.
Working together with community, mental health, social care, pharmacy, hospital, voluntary, community and social enterprise services, the aim is to offer more personalised, coordinated health and social care.
